Nurse Care Coordinator

The Nurse Care Coordinator at Kaiser Permanente plays a vital role in ensuring that patients receive coordinated and compassionate care throughout their journey. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement effective care plans.
Key Responsibilities:- Conduct thorough assessments to identify patient needs and preferences.
- Develop and update individualized care plans in collaboration with healthcare teams.
- Coordinate care transitions, including hospitalizations, appointments, and tests.
- Foster open communication with patients, families, and caregivers to ensure informed decision-making and active engagement in care planning.
- Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire.
- Registered Nurse License (Oregon) within 6 months of hire.
- Baccalaureate degree in nursing or commensurate knowledge and experience.
- Minimum two years of experience in medical-surgical in an acute care setting or two to four years of experience in home health or infusion therapy.
$80,000 - $105,000 per year, depending on experience.
