Transitional Foster Care Case Coordinator

Position Overview:
This role is designed for a dedicated individual to provide essential case management and support services to children in foster care, as well as their families. The coordinator will play a vital role in ensuring the well-being of unaccompanied minors and their integration into safe environments.

Key Responsibilities:
  • Conduct comprehensive initial assessments to ensure children feel secure and are well-informed upon arrival.
  • Facilitate prompt communication with youth sponsors and ensure all necessary documentation is completed.
  • Collaborate with consulates to identify suitable sponsors for children arriving without known contacts.
  • Utilize a trauma-informed approach to expedite the Family Reunification Packet process, ensuring safe transitions for youth.
  • Guarantee that all youth receive essential services, including mental health, medical, educational, and recreational support.
  • Maintain accurate records and documentation in compliance with organizational and regulatory standards.
  • Lead treatment team meetings in partnership with therapists to discuss youth progress and needs.
  • Engage in weekly staff meetings to collaborate with the treatment team and stakeholders.
  • Provide ongoing education and support to foster parents to enhance their caregiving capabilities.
  • Conduct monthly meetings with foster families to address any service delivery concerns.
  • Assist in translating communications for program participants and staff when necessary.
  • Maintain effective relationships with community agencies involved in service provision.
  • Participate in training sessions to enhance service delivery skills.
  • Prepare for and attend court hearings as required.
  • Transport clients as needed to ensure access to services.
  • Stay informed about community resources and refer families for additional support.
  • Adhere to all relevant agency, federal, and state regulations related to social services.
  • Perform other duties as assigned.
Qualifications:
  • Bachelor's degree in Behavioral Sciences, Human Services, or a related field from an accredited institution.
  • Experience in child welfare or case management is highly desirable.
  • Strong understanding of social work principles and methodologies.
  • Fluency in English and Spanish is preferred, along with experience working with refugee or immigrant populations.
  • Demonstrated cultural sensitivity and mature judgment.
  • Excellent communication skills, both verbal and written.
  • Proven ability in clinical, therapeutic, and crisis intervention practices.
  • Ability to work independently while maintaining confidentiality.
  • Proficient in Microsoft Office and other relevant software.
  • Must be at least 21 years old with a valid driver's license and a clean driving record.
  • Successful completion of a criminal background check and child protection agency screenings.
  • Willingness to comply with health requirements regarding vaccinations.
  • Commitment to the organization's mission and values.
