Job Description
We are seeking a skilled Patient Care Manager to join our team at Georgia Home Health, a part of LHC Group. As a Patient Care Manager, you will be responsible for the supervision and coordination of clinical services, providing quality support to patient-facing teams, and ensuring that patients receive the best possible care in the comfort of their own homes.

Responsibilities
Supervise and coordinate clinical services to ensure high-quality patient care
Provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing
Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits
Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals
Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders

Qualifications
Current RN licensure in the state of practice
Current CPR certification required
Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
