Care Manager

Responsibilities:
- Coordinate patient care plans to ensure seamless transitions between healthcare settings.
- Develop and implement discharge plans to promote timely and safe patient discharges.
- Collaborate with healthcare professionals to identify and address patient needs.
- Monitor patient progress and adjust care plans as needed.
Qualifications:
- Graduate of a Nursing Program.
- Two (2) years of experience in an acute or post-acute setting.
- Current unrestricted registered nurse (RN) license, or eligible for licensure, in the state of Georgia.
Preferred Qualifications:
- Certification in Case or Care Management.
- Experience using InterQual or Milliman.
- Masters degree.
