We are seeking an experienced MDS Coordinator to join our team and contribute to the exceptional care we provide to our patients.

About the Position:

This is a key role that requires strong organizational, communication, and leadership skills. As an in-house case manager, you will be responsible for coordinating services with physicians, families, third-party payers, and facility staff to ensure seamless resident care.

Key Responsibilities:
  1. Accurately complete the Resident Assessment Instrument according to federal and state regulations and company policies.
  2. Collaborate with healthcare professionals, families, and support teams to develop comprehensive care plans tailored to each resident's needs.
Requirements:
  • Current licensure as a Licensed Practical Nurse or Registered Nurse in the state of practice.
  • At least two years of clinical experience in a healthcare setting, preferably in long-term care.
  • Excellent problem-solving, time management, and interpersonal skills.
Compensation Package:
