About the Role

We are seeking a skilled Senior Care Coordinator to join our team at Brickyard Healthcare. This is an exciting opportunity for an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to take on a challenging role in long-term care.

As a Senior Care Coordinator, you will be responsible for ensuring that patients receive the necessary care and support while maintaining effective resource management. Your key tasks will include completing the Resident Assessment Instrument and coordinating care services with various stakeholders, including physicians, families, third-party payers, and facility staff.

Responsibilities
  • Resident Assessment and Care Planning: Complete the Resident Assessment Instrument per federal and state regulations and company policy and procedures.
  • Care Coordination: Act as an in-house case manager by considering all aspects of the resident's care and coordinating services with relevant parties.
Requirements
  • Licensure and Certification: Current, active license as an LPN or RN in the state of practice and current CPR certification.
  • Clinical Experience: Minimum two (2) years of clinical experience in a healthcare setting, with a minimum of one (1) year in long-term care.
  • Prior Experience: Prior experience as an MDS coordinator with demonstrated expertise.
