Job Description

As an MDS Coordinator, you will be responsible for ensuring that our residents receive the highest level of care. This includes:

  • Completing resident assessments in accordance with federal and state regulations
  • Coordinating services with physicians, families, third-party payers, and facility staff
  • Developing and implementing individualized care plans

We require a Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice. You should also have a minimum of two years of clinical experience in a healthcare setting and prior experience as an MDS coordinator. A strong understanding of long-term care principles and practices is essential.
