The St. Charles Health System Pharmacy delivers a comprehensive array of inpatient and outpatient pharmaceutical care services to guarantee safe and effective medication use.


POSITION OVERVIEW:

The Pharmaceutical Care Specialist delivers comprehensive pharmaceutical services throughout the hospital medication process to enhance the effectiveness and quality of patient care.

This role does not directly supervise any other staff members.

ESSENTIAL FUNCTIONS AND DUTIES:


Provides specialized pharmaceutical services aimed at improving medication utilization and therapeutic outcomes, including advising healthcare providers on drug therapy issues, potential toxicity, and side effects, as well as assisting in determining appropriate dosages.

Coordinates the efforts of pharmacy technicians involved in the preparation, labeling, and distribution of medications and pharmaceutical supplies in accordance with medical prescriptions; ensures adherence to federal, state, and local regulations regarding drug dispensing and the maintenance of necessary records.

Mentors and trains individuals in training programs, including practical experiences.

Maintains collaborative relationships with medical and nursing personnel; provides timely information regarding pharmaceutical supplies, medication usage, compatibility, and compliance with state and federal drug control regulations and Joint Commission Standards.

Conducts and assesses medication histories, evaluates compliance, and recommends modifications to achieve desired outcomes; instructs patients on the proper use of prescribed medications and participates in Patient Care Rounds with healthcare providers to monitor patient progress.

Customizes medication regimens based on sound principles, considering pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination.
Extracts relevant information from patients and other sources for the patient's medical management plan.
Operates pharmacy computer systems and other necessary equipment.
Exemplifies effective communication of the St. Charles Health System mission, vision, values, and strategic objectives for the department.
Supports Lean principles of continuous improvement with enthusiasm, acting as a champion for change.
Ensures a safe environment for staff, patients, and visitors.
Conducts all activities with the utmost professionalism and confidentiality. Complies with all applicable laws, regulations, policies, and procedures, supporting the organization's corporate integrity initiatives.

EDUCATION

Required:
Bachelor of Science degree in Pharmacy.

Preferred:
PharmD., Completion of an ASHP accredited PGY-1 residency training
LICENSURE/CERTIFICATION/REGISTRATION

Required:
Current registration by the Oregon Board of Pharmacy.

Preferred:
BCPS certification.
