Long Term Care Unit
Helen Porter continues to build upon its 30-year tradition of high-quality long-term care services, providing nursing care and assistance with daily living, as well as programs and activities tailored to residents' abilities and needs.
Memory Care Unit
The Memory Care Unit is designed to meet the changing needs of individuals with dementia, offering activity-based care in a warm, supportive, and safe environment, with a primary goal of helping individuals maintain dignity, purpose, and functional ability.
Post Acute Care
The Post Acute Unit at UVMHN Helen Porter Rehabilitation & Nursing provides supportive, goal-directed care following surgery and/or hospitalization, with specially designed treatment programs enabling patients to return home as quickly as possible, reaching their highest possible level of independence.
Routine Therapy services are provided Monday through Friday, with the capability of providing therapy six days a week if required to maximize positive results. Nursing staff provide necessary extensions of therapies when therapy staff are not present to provide direct service.
*Subject to applicable taxes, the bonus is paid in three installments: 1st after 90 days, 2nd after one year, and 3rd after two years. Bonus comes with a commitment period equal to the length of payments and requires an agreement be signed.
