Position Summary: This role involves delivering perinatal outreach services to a targeted demographic within the community. The selected candidate will provide comprehensive case management services, ensuring clients have access to essential resources such as medical, nutritional, social, educational, and substance abuse support. Daily interactions with clients, either in person or via phone, will focus on enhancing the health and well-being of both mothers and their infants.
Key Responsibilities:
• Deliver case management for clients facing psychosocial challenges through home visits and group settings.
• Coordinate and collaborate with service providers, ensuring timely referrals and follow-ups.
• Conduct perinatal depression screenings for all mothers enrolled in the program.
• Provide extensive education on interconception health and ongoing support for child development to clients and their families.
• Assist in arranging transportation and encourage attendance at healthcare appointments, educational sessions, and social service meetings.
• Facilitate non-clinical group discussions and provide support for health-related classes for clients and alumni.
• Maintain accurate documentation of case management activities and prepare necessary reports.
• Ensure all records and forms are complete and accurate.
• Participate as a member of a multidisciplinary Care Coordination team to provide holistic client care.
• Attend community council meetings and report on activities.
• Travel as needed for home visits and community engagement, documenting mileage for reimbursement.
• Engage in research studies as assigned.
Required Skills and Qualifications:
• Strong communication skills to engage with diverse constituents and effectively prioritize tasks in a dynamic environment.
• Ability to manage a caseload, organize work assignments, and make informed decisions in high-pressure situations.
• Reliable transportation and a valid driver's license, with the ability to travel flexibly for home visits and community outreach.
• Bilingual (English/Spanish) proficiency is required for certain positions.
Education and Experience:
• Bachelor’s degree in social work, human services, or a related field from an accredited program focusing on maternal and child welfare.
• Proficient in Microsoft Office Suite and other relevant software applications.
Physical Requirements:
The role requires light physical activity typical of administrative tasks, including:
• Light lifting/moving of items up to 25 lbs.
• Manual dexterity for handling materials and equipment.
• Adequate vision for close, distance, peripheral, and depth perception.
