Job Description

We are seeking two Full-Time Treatment Coordinator/QIDPs for our Manhattan ICF residences. The annual salary for this position is between $55-$65K, plus a comprehensive benefits package.

Key Responsibilities
  • Coordinate the training and habilitation program of assigned residents with residential staff, educational and vocational rehabilitation programs, and other needed services.
  • Generate Needs/Capabilities lists annually, with updates as needed, to ensure each assigned client has an activity schedule, ongoing continuous active treatment, and implementation of an activity schedule as documented.
  • Act as resident advocate in areas related to resident cleanliness, ensuring adequate clothing, personalized and clean bedrooms, toiletries, etc.
  • Complete reviews of each assigned resident's individual treatment plan by summarizing progress or need for program modification as supported by concrete, measurable data obtained by direct care staff.
  • Act as liaison with day/workshop programs, ensuring coordination of services between day and residential components.
  • Ensure that IPPs and day program progress notes are received in timely fashion and remain on file.
  • Ensure that direct care/clinical staff are trained in behavioral interventions and active treatment areas, so that all staff working with individuals are able to carry out treatment plans as written.
  • Coordinate the Annual Review process by ensuring reports from all disciplines are collected in timely manner, preparing annual reports, and documenting rationale for type of intervention upon needs addressed in upcoming year.
Qualifications
  • Bachelor's Degree in a human services-related field.
  • Basic knowledge of all clinical areas that comprise the interdisciplinary team for individuals with intellectual disabilities.
  • Good working knowledge of direct care responsibilities and ability to perform such duties.
  • Leadership abilities and ability to coordinate a team of clinical and management staff.
  • Excellent organizational skills and good writing ability.
  • Ability to deal tactfully, professionally, and effectively with residents, personnel, family members, visitors, government agencies, and the general public.
  • Willingness to work flexible hours.
