Job Title: Rehabilitation Counselor

At Constructive Partnerships Unlimited, we are seeking a highly motivated and compassionate Rehabilitation Counselor to join our team. As a key member of our multidisciplinary clinical team, you will play a vital role in providing direct services to individuals with intellectual and developmental disabilities.

Job Summary:

The Rehabilitation Counselor will be responsible for implementing strategies contained within behavior intervention plans, providing training, advocacy, and treatment to individuals served, and collaborating with a multi-disciplinary team to develop and implement individualized comprehensive treatment plans.

Key Responsibilities:
  • Implement behavior intervention plans and provide training, advocacy, and treatment to individuals served.
  • Collaborate with a multi-disciplinary team to develop and implement individualized comprehensive treatment plans.
  • Teach lessons on daily living skills, including medication administration, exercise and nutrition training, community integration, human rights advocacy, social and life-skills training, and educational and life planning.
  • Assist and actively participate in all aspects of the Life Plan or Comprehensive Functional Assessment (CFA) and ensure residents' plans of care are implemented.
  • Transport individuals served by driving the agency vehicle and/or accompanying them on appointments or scheduled activities.
  • Assist and/or support daily living skills, including personal hygiene, food preparation, housekeeping, laundry, shopping, and ensure residents are appropriately dressed at all times.
  • Advocate for or provide support to individuals in their home and while in the community, and maintain safety for individuals served while accompanying them outside of the home.
  • Facilitate learning and skill training in fire safety and safely evacuate individuals who cannot do it independently.
  • Correctly apply or assist in the application of personal adaptive equipment and report any damaged, missing, or defective adaptive equipment or environmental safety hazards to a Supervisor/Manager or a Nurse.
  • Report any suspicion of verbal, physical, psychological, sexual abuse or neglect of individuals served to the Justice Center, Supervisor, Manager, QIDP, BIS, or Nurse.
  • Read data/documentation and report errors or missing data to the Supervisor/Manager and write appropriate data during the assigned shift.
  • Apply knowledge and skills gained through required training, including medication administration, CPR, and Strategies for Crisis Intervention & Prevention-Revised (SCIP-R).
  • Perform assigned household chores, including cooking, cleaning, laundry, food shopping, and maintenance-related duties performed both inside and outside of the residence.
  • Assist in maintaining an inventory of residents' clothing and personal effects on a routine basis to ensure individuals are properly clothed.
  • Participate in the resolution of crisis management and report all incidents, both verbally and in writing, to the supervisor/manager.
  • Perform bed checks, rounds, and repositioning of residents every two hours, or as determined to be necessary by individual plan, or as assigned.
Requirements:
  • BS/BA degree in Education, Psychology, Sociology, or Social Work.
  • A minimum of one year of experience working with individuals with developmental disabilities.
  • Valid and clean driver's license for at least 3 years and successful completion and maintenance of CP's Driver Authorization.
  • Ability to work cooperatively with team members and all other Agency personnel.
  • Strong communication and customer service skills and ability to maintain confidentiality.
  • Respectful of the rights and feelings of others and maintains a demeanor appropriate to a work environment.
  • Must successfully complete within three months of hire date, and then maintain, certification for required training in Medication Administration, and Strategies for Crisis Intervention & Prevention-Revised (SCIP-R) as required.
