About the Role

At UnitedHealthcare, we're committed to simplifying the healthcare experience and creating healthier communities. As a Field Health Social Services Coordinator/IDD, you'll play a critical role in removing barriers to quality care and making a meaningful impact on the lives of millions of people.

This is a field-based role with a home-based office, offering the flexibility to work remotely within a designated area. You'll be responsible for assessing, planning, and implementing care strategies that cater to individual patient needs, while ensuring the most appropriate level of care is provided.

Key Responsibilities
  • Develop and implement personalized care plans that address the unique needs of each patient
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate effectively with stakeholders to ensure timely and coordinated care
  • Advocate for patients and families to ensure their needs and choices are represented and supported
Requirements
  • Licensed Master's Social Worker (LMSW), Licensed Professional Counselor (LPC), or Licensed Bachelor Social Worker (LBSW) OR RN in the state of Texas
  • 2+ years of experience working directly with individuals with Intellectual and Developmental Disability (IDD)
  • 2+ years of experience in a community health setting or healthcare role
  • Familiarity with Microsoft Office, including Word, Excel, and Outlook
  • Reliable transportation and ability to travel within the assigned region
Preferred Qualifications
  • 1+ years of experience with long-term care services and support, Medicaid, or Medicare
  • Ability to create, edit, save, and send documents, spreadsheets, and emails
  • Knowledge of principles of most integrated settings, including federal and state requirements
