Responsibilities:
As a Case Coordinator, you will be responsible for:
- Providing case management and behavioral support to individuals with developmental disabilities
- Developing and implementing individualized support plans to promote independence and self-advocacy
- Collaborating with the clinical team to provide comprehensive support services
- Documenting progress and outcomes in a timely and accurate manner
Qualifications:
A bachelor's degree in a related field and experience working with individuals with developmental disabilities are required. Training in applied behavior analysis and first aid/CPR certification are preferred. Skilled in the use of Microsoft Word and a valid driver's license are also required.
