Overview

Responsibilities

Under the direction of the Manager, Case Coordination, the RN-Case Coordinator will perform all functions of a case manager, coordinating the care and services of assigned patients along the continuum of care. This includes working collaboratively with physicians and multidiscipline team members to improve patient care through effective utilization and monitoring of healthcare resources.

The RN-Case Coordinator will facilitate patient/family/representative achievement of desired outcomes and possess strong clinical knowledge, creative problem-solving skills, and multitasking abilities.

Requirements

A Bachelor's Degree in Nursing or a related field is required, along with a current Massachusetts RN license and certification in Case Management. Over five years of related experience is preferred, with one year of case management experience being ideal.

The successful candidate will possess excellent time management and organizational skills, with the ability to tolerate interruptions and a demanding work assignment. Knowledge of funding, resources, services, clinical standards, and outcomes is crucial to this role.
