Key responsibilities include facilitating patient/family/representative achievement of desired outcomes, working under the direction of the Manager, Case Coordination, and possessing strong clinical knowledge and multitasking skills.

Requirements include a Bachelor's Degree in Nursing or a related field, current Massachusetts RN license, and over five years of related experience. Certification in Case Management is preferred.
