Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice RN Case Manager. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients and their families.

Responsibilities
  • Perform initial nursing assessments on new admissions and develop individualized care plans to ensure the best possible outcomes.
  • Collaborate with the care team to coordinate services, including referrals to other hospice care disciplines as needed.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
  • Develop, prepare, and maintain accurate and timely patient care progress records.
  • Provide education to families on patient condition, needs, and care.
  • Perform on-call responsibilities and provide services to patients and families as assigned.
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of nursing experience preferred, or consideration for the RN new graduate program.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
