About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team at SouthernCare, a leading provider of hospice care services. As a Hospice RN, you will play a vital role in delivering high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to their families.
  • Act as a liaison between physicians, patients, and the hospice team.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care.
  • Manage and collaborate with an interdisciplinary team to provide comfort, peace, and dignity to patients and their families.
  • Maintain accurate and up-to-date patient clinical records.
Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Current drivers license and automobile insurance.
  • Current CPR certification.
