Responsibilities

We're seeking a skilled Patient Services Coordinator to join our team. As a key member of our care coordination team, you'll be responsible for scheduling patients to branch field clinicians, communicating with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. You'll also manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

Qualifications

To be successful in this role, you'll need a high school diploma or equivalent, and either a licensed practical or vocational nurse in the state in which you currently practice, with at least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months, with a demonstrated understanding of staffing and scheduling requirements related to home care services. You'll also need basic demonstrated technology skills, including operation of a mobile device.
