Position Overview:

The Clinical Care Coordinator plays a vital role in managing patient transitions and overseeing the duration of hospital stays while ensuring high-quality care, patient safety, and financial accountability. This position promotes a cohesive care model, coordinates patient flow, and encourages collaboration among healthcare teams, as demonstrated by performance metrics and best practices.

As a member of the Care Management team, the Clinical Care Coordinator is responsible for facilitating the discharge planning process from the moment of admission through to discharge. This role may also involve intervening with patients who present complex or high-risk psychosocial needs.

Key Responsibilities:
  • Facilitate effective discharge planning and patient throughput.
  • Collaborate with healthcare teams to ensure seamless care delivery.
  • Monitor and optimize performance metrics related to patient care.
  • Support patients with complex psychosocial needs as necessary.
Qualifications:

Education: Bachelor's Degree in Nursing.

Licensure: Current nursing license in the state of employment is required.

Experience: 2-3 years in an acute care hospital setting is preferred, along with experience in Care Management or Utilization Management.

Certifications: BLS (CPR) certification required at hire or within 90 days; certification in Case Management is preferred.

Skills and Knowledge:
  • Exceptional verbal and written communication abilities.
  • Strong assessment, time management, and critical thinking skills.
  • Proficient in computer applications.
  • Able to interact effectively with diverse individuals and situations.
  • Knowledge of age-specific care requirements, including neonatal, pediatric, adult, and geriatric populations.
  • Familiarity with InterQual or Milliman Care Guidelines is preferred.
