Position Overview:
The Clinical Care Coordinator at United Health Services is a vital role within our Care Management department. This position is designed to enhance patient care by navigating and coordinating services across various healthcare settings.
Responsibilities:
The Clinical Care Coordinator will:
- Oversee and facilitate the care of patients throughout their healthcare journey.
- Monitor and manage resources effectively to ensure patients transition to the appropriate level of care.
- Assess, plan, coordinate, and evaluate patients' care plans in collaboration with the healthcare team.
- Collect and analyze data to improve patient care processes and outcomes.
- Educate stakeholders, including payers and providers, on healthcare trends and initiatives that enhance care delivery.
Qualifications:
Education/Experience:
Minimum of an Associate's degree in Nursing with at least two years of experience in an acute or tertiary care setting. A Bachelor's degree in Nursing and five years of experience are preferred.
Licensure/Certification:
Must possess a valid New York State Registered Nurse license.
