Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at St Croix Hospice. As a key member of our interdisciplinary team, you will provide comprehensive hospice care services to patients and their families.

Key Responsibilities
  • Provide medical care and palliative services to patients with terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
  • Evaluate patient health history, health promotion, diagnose medical conditions, manage health problems, and develop individualized plans of care.
  • Prescribe medications and treatments, coordinate and collaborate with patients, families, and other healthcare practitioners to ensure seamless care.
  • Visit patients, complete physical assessments, perform face-to-face assessments, and evaluate hospice eligibility as assigned in consultation with the hospice interdisciplinary team.
  • Build relationships with patients, families, and community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.
Requirements
  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Current nursing licensure in State and CPR certification.
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics).
