Palliative Care Nurse Practitioner

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Amedisys. As a Palliative Care Nurse Practitioner, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on alleviating symptoms and improving quality of life.

  • Provide palliative medical services, managing symptoms and developing treatment plans in collaboration with physicians.
  • Work independently and in collaboration with physicians to deliver palliative care in various settings, including home, outpatient clinics, hospitals, and skilled nursing facilities.
  • Order treatments, laboratory, and diagnostic services, and coordinate care with patient's healthcare team through effective communication and planning.
Responsibilities
  • Clinical Patient Care Management: Provide palliative care for residents in their home or clinical setting, collaborating with physicians and/or medical directors as required by state and federal law.
  • Performs medically necessary assessments, accurately and timely documenting Evaluation and Management (E&M) codes.
  • Schedules visits based on medical assessment and individualized care plans, orders and reviews treatments, lab, x-rays, and other diagnostic procedures as indicated by a palliative plan of care.
Qualifications
  • Required: One (1+) year experience as a Nurse Practitioner or three (3+) years' experience as a Registered Nurse in geriatric, hospice, or palliative care.
  • Master's Degree or higher in Nursing.
  • Licensed Nurse Practitioner in the state where services are provided.
  • Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
