Palliative Care Nurse Practitioner Job Description

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Amedisys. As a Palliative Care Nurse Practitioner, you will play a vital role in delivering high-quality patient care and collaborating with healthcare professionals to ensure the best possible outcomes for our patients.

Key Responsibilities:
  • Provide palliative care services to patients in their homes or clinical settings, working collaboratively with physicians and medical directors to develop and implement individualized care plans.
  • Perform medically necessary assessments, including evaluations and management of patients' conditions, and document findings accurately and timely.
  • Order and review diagnostic procedures, such as lab tests and imaging studies, and communicate results with collaborating physicians as needed.
  • Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to ensure seamless care coordination and delivery.
  • Develop and maintain relationships with referral sources, including hospitals, assisted living facilities, and primary care providers, to ensure optimal patient care.
Qualifications:
  • Master's degree in Nursing or higher.
  • Current licensure as a Nurse Practitioner in the state where services will be provided.
  • Certification as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Authority to practice and prescribe as a Nurse Practitioner in the state where services will be provided, if permitted by state and federal law.
  • Medicare billing privileges as a Nurse Practitioner.
  • Current CPR certification.
