SUMMARY
The South Middlesex Opportunity's Young Parent Living Program is designed to support single young parents aged 14-22 and their children. The Case Manager plays a crucial role in assisting families by creating and implementing a comprehensive program that encompasses individual evaluations, service planning, educational guidance, and group activities, alongside enrichment and educational initiatives.
KEY RESPONSIBILITIES
- Guide families in accessing necessary services, establishing objectives, and formulating a strategy to achieve those objectives.
- Facilitate conflict resolution between families and young individuals.
- Demonstrate effective parenting techniques in both structured and casual settings.
- Comply with program protocols.
- Collaborate with external agencies and resources to secure long-term services as required (such as childcare, education, healthcare, counseling, etc.).
- Maintain precise documentation for each family.
- Engage in self-assessment to identify and pursue professional development goals.
- Contribute to maintaining a safe and clean environment within the program.
- Hold a valid driver's license to provide transportation for clients.
- Conduct family intake assessments.
- Participate in on-call duties.
- Enhance collaborative relationships with key resources in the client network.
- Attend staff meetings and lead house meetings.
- Complete agency-mandated training sessions.
- Engage all clients by recognizing and addressing their needs, whether within or beyond the scope of the specialty.
- Participate in engagement team meetings, follow up on client service plans, and communicate effectively with clients and staff across various specialty areas.
- Ensure adherence to program, departmental, agency, and funder standards, as well as SMOC policies and procedures.
- Perform additional duties as assigned.
QUALIFICATIONS AND SKILLS
- Experience working with homeless individuals and/or teenagers is preferred.
- Excellent verbal and written communication abilities.
- Bi-lingual candidates are preferred.
- A Bachelor's degree in counseling, social work, psychology, or a related field is required, or equivalent training as a paraprofessional with a minimum of three years of experience.
- A valid driver's license is mandatory.
ORGANIZATIONAL STRUCTURE
- This position reports directly to the Program Director.
- There are no direct reports for this role.
WORK ENVIRONMENT
The program operates from a multi-story residential facility. Staff members are expected to conduct regular checks and respond to the needs of guests across all levels. Minimal physical lifting is required. Driving is an essential duty, as the program is responsible for transporting all referrals. As part of this role, the Case Manager will have direct or incidental contact with clients served by SMOC across various programs funded or managed through the Executive Office of Health and Human Services. A background check is required.
