About the Position:

This position is part of YAI's Community Habilitation services in Rockland County, New York. As a Community Habilitation staff member, you will provide one-on-one support to individuals with intellectual and developmental disabilities. You play an integral role in teaching various life skills while providing recreational opportunities to promote community integration.

The goal of this program is to empower individuals we support to obtain greater independence. This involves providing individualized supports that cater to each person's unique needs and abilities. As a Community Habilitation staff member, your responsibilities will include adhering to policies and procedures, using effective communication skills, providing one-to-one supports in community and home settings, completing mandatory trainings, and meeting OPWDD Core Competencies. Additionally, you will be responsible for ensuring health, wellness, and safety by managing crisis situations and providing appropriate supervision of the people we support.

You will also be required to transport and escort individuals on recreational activities, medical appointments, home visits, etc., and build positive relationships with the people we support. Maintaining a therapeutic environment through various methods such as Personal Outcome Measures, Person Centered Thinking, Informed Decision Making is crucial in this role. Furthermore, you will be responsible for ensuring person-centered plans are implemented and completing documentation to ensure compliance and billing.

To succeed in this role, you should have high school diploma or GED preferred, work or life experience within the intellectual/developmental disabilities field, and basic written and verbal communication. Basic computer knowledge and valid driver's license may be required depending upon the position. Tuberculosis test is mandatory for most positions. Interpersonal and problem-solving skills are essential for this role.
