Empower the Lives of People with Disabilities

We are seeking a dedicated Community Habilitation Specialist to join our team at ADAPT Community Network, formerly United Cerebral Palsy (UCP) of NYC. This is a unique opportunity to make a meaningful difference in the lives of individuals with developmental disabilities.

About the Role:

As a Community Habilitation Specialist, you will be responsible for providing ongoing training and personal assistance to participants with disabilities, promoting their independence and community inclusion. Your work will involve developing individualized habilitation plans, providing support and guidance in daily living skills, and fostering a sense of empowerment and autonomy.

Key Responsibilities:
  • Adhere to ethical principles, including the agency's mission statement and standards of conduct.
  • Support participants in choosing and participating in recreational, vocational, and daily living skills activities, based on their desires, program parameters, and individualized plans.
  • Assess participant interests, abilities, strengths, and limitations.
  • Follow dietary guidelines as directed in habilitation plans.
  • Provide personal care consistent with participant skill levels and plan outlines.
  • Demonstrate sensitivity and responsiveness to participants.
  • Work with supervisors and colleagues to meet program needs.
  • Develop effective relationships with participants' support networks.
  • Maintain confidentiality in accordance with HIPPA regulations.
  • Collaborate with other service providers in the community.
  • Participate in regular staff meetings and required trainings.
Requirements:
  • Ability to read, write, and speak English fluently.
  • Computer literacy sufficient to accommodate Electronic Medical Record software training.
  • Able to implement prompting hierarchies and document session notes.
  • Flexible schedule to accommodate participant needs.
  • Will to travel to participants' homes or meet them in the community.
  • Successful completion of required in-service training.
  • Background checks through designated state agencies.
