Position Title: Community Care Coordinator I

Company Overview: The Philadelphia Corporation for Aging is seeking a dedicated Community Care Coordinator I to oversee a variety of client caseloads and deliver essential long-term care services to elderly participants within the community. This role emphasizes advocacy for participants, ensuring the provision of high-quality service.

Qualifications:

Educational Background: A Bachelor’s degree in Social Work (BSW), Social Sciences, or a Master’s degree in Social Work (MSW) with relevant coursework in gerontology is required.

Professional Experience: A minimum of one year of experience in social work is necessary.

Objectives:

To deliver exceptional care management services by addressing the needs of participants and facilitating home and community-based services that encourage independence.

Responsibilities:

  • Perform comprehensive care management, including assessment, planning, coordination, and monitoring of services.
  • Create personalized service plans and engage participants in the decision-making process.
  • Coordinate services and benefits, ensuring optimal alignment with participant needs.
  • Evaluate participant satisfaction and the quality of services provided.
  • Accurately complete necessary documentation and data entry in a timely manner.
  • Engage in training and professional development activities.
  • Fulfill other assigned responsibilities as needed.

Performance Standards:

  • Comply with agency standards and best practices.
  • Build effective relationships with stakeholders.
  • Identify challenges and collaboratively develop solutions.
  • Provide quality services to participants.

Target Audience: Participants, service providers, and PCA staff.

Compensation Range: $42,000 - $49,310.45
