Job Title: Case Manager

PATH Integrated Health is seeking a skilled Case Manager to join our dynamic team. As a Case Manager, you will play a vital role in providing comprehensive care to our clients, ensuring they receive the support and resources they need to achieve their goals.

Key Responsibilities:
  • Act as a liaison between care team members and clients' families, ensuring seamless communication and coordination of care.
  • Arrange family visits, coordinate transportation, and ensure clients' needs are met in a timely and efficient manner.
  • Maintain clear and ongoing communication between the family, referral source, and PATH Integrated Health, ensuring a collaborative and supportive care environment.
  • Engage in a professional and collaborative case management process to support clients' mental and physical health needs, promoting optimal outcomes and quality of life.
  • Assess clients' individual needs, developing personalized care plans tailored to their unique circumstances and goals.
  • Coordinate and collaborate with other care team members to ensure seamless delivery of care and support, enhancing clients' well-being and quality of life.
  • Monitor clients' progress regularly, adjusting care plans as needed to address evolving needs and promote optimal outcomes.
Qualifications:
  • Candidate must have a High School Diploma or GED. Bachelor's degree is preferred.
  • Candidates must have a minimum of 1 year of case management experience.
  • Excellent verbal and written communication skills, with the ability to communicate professionally via telephone and in writing.
  • Strong grammar and writing skills, with the ability to clearly and concisely communicate with clients, colleagues, and other healthcare professionals.
  • Ability to work under stressful conditions and to work irregular hours, with a strong commitment to providing high-quality care and support.
