Job Summary

We are seeking a highly motivated and compassionate Shelter Case Manager to join our team at Health Equity International. As a key member of our staff, you will play a vital role in providing intensive case management services to clients and their families residing at emergency shelters in Lexington and Woburn.

Key Responsibilities
  • Conduct thorough assessments of client service needs and develop individualized self-sufficiency plans to enhance their housing situation.
  • Provide guidance and support to clients in navigating the housing and employment systems, including connecting them with local resources and services.
  • Collaborate with other social service providers and municipal officials to ensure a comprehensive approach to addressing client needs.
  • Document client progress and maintain accurate and timely case notes and service plans.
  • Transport clients as needed and participate in weekly housing meetings to present client cases for permanent housing opportunities.
Requirements
  • Bachelor's Degree or a minimum of three years' experience in Human Services or a related field.
  • Haitian Creole and/or Spanish language proficiency is preferred but not required.
  • Sensitivity to low-income individuals of diverse backgrounds and cultures.
  • Assessment, advocacy, and case management skills.
  • Reliable transportation, valid Massachusetts driver's license, and ability to meet insurance requirements.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with individuals from diverse backgrounds and cultures.
