Pediatric Nurse Case Manager Opportunity

We are seeking a skilled Pediatric Nurse Case Manager to join our team at The Kidz Club PPEC. As a key member of our multidisciplinary team, you will be responsible for managing the care of pediatric patients, ensuring their individual needs are met through a holistic approach.

Key Responsibilities:
  • Manage patient care plans, collaborating with pediatricians, therapists, and family members
  • Facilitate referrals to primary providers, specialists, and community resources
  • Provide direct client care, including educational, psychosocial, behavioral, physical, mental, and emotional support
  • Educate clients and families on immediate care needs and preventative behaviors
Requirements:
  • Graduate of an accredited school of nursing with current RN licensure eligible in the state of Kentucky
  • Recent graduate with a provisional RN license
  • Basic personal computer skills and comfort with learning electronic charting systems
  • Preferred: At least six months of current experience as a practicing RN with pediatric experience
