We are seeking a compassionate and skilled Hospice Care Assistant to join our team at Partners In Care. As a Hospice Care Assistant, you will provide personal care to patients in our Hospice House, ensuring their comfort and dignity. Your role will involve assessing patient needs, coordinating with care managers, and reporting any changes in patient condition.

Responsibilities:

  • Provide personal care to patients, including bathing, dressing, and grooming.
  • Assess patient needs and coordinate with care managers to ensure accurate care plans.
  • Report any changes in patient condition to care managers and document accordingly.
  • Prepare meals and assist with feeding patients as needed.
  • Perform housekeeping services to maintain a safe and healthy environment.

Requirements:

  • Valid CNA license.
  • Ability to pass a background test, drug screen, and TB test.
  • Graduate of an accredited nursing school.
  • One to two years of recent medical care experience in an institutional setting.
  • Current automobile insurance and a valid Oregon driver's license.
  • Valid CPR certification.
  • Strong communication and interpersonal skills.
