Position Overview:

As a Client Relations Specialist at Heart to Heart Hospice, you will play a pivotal role in fostering a patient-centered approach within the healthcare ecosystem, particularly in collaboration with healthcare facilities and hospitals.

Key Responsibilities:

  1. Facilitate seamless coordination of care among healthcare professionals to ensure optimal patient outcomes.
  2. Oversee the ordering, delivery, and maintenance of Durable Medical Equipment (DME), ensuring that patients and their families receive comprehensive training on its use.
  3. Engage in regular communication with patients and their families, addressing their needs and ensuring that Heart to Heart Hospice consistently delivers exceptional service.
  4. Identify and resolve any issues or dissatisfaction expressed by patients or family members, effectively communicating with the clinical team and management as necessary.
  5. Organize and conduct educational sessions or in-services for family members and healthcare facilities to enhance understanding and support.

Qualifications:

To be successful in this role, candidates should possess:

  • A diploma, associate, or bachelor’s degree from an accredited school of vocational nursing.
  • A current state license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) is preferred.
  • A valid driver’s license and car insurance.
  • A minimum of two years of experience as an LVN or LPN in a clinical care environment, with a preference for hospice or home health experience.
  • Strong background in customer service is highly desirable.
