Care Coordinator

About the Role:
Gastro Health is seeking a Part-Time Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the smooth operation of our medical services. Your primary responsibility will be to handle all administrative tasks and duties for the physician/provider, serving as the liaison or coordinator for the patients medical care. You will also be responsible for streamlining all patient-physician communications to ensure patient satisfaction, providing medical literature and clinical preparation instructions to patients, and assisting patients with questions and/or concerns regarding procedures. Additionally, you will schedule all procedures to be performed by the physician, review the physicians schedule for maximum scheduling efficiency, and schedule all diagnostic tests, procedures, and follow-up appointments. You will also obtain all authorizations for procedures and tests, call patients to confirm procedures a week in advance, and schedule follow-up appointments including recalls. Furthermore, you will check-out patients at the end of their visit and provide next step instructions, request medical records from doctors and hospitals, return patient calls promptly and professionally, call-in new prescriptions and refills and obtain authorization if necessary, and obtain lab results including stat requests. You will also complete tasks from Electronic Medical Record, review open orders every three days and work accordingly, contact patients with test results, and send history and physical forms to outpatient facility. Other duties as assigned.
Requirements:
High school diploma or GED equivalent. Certified Medical Assistant (AAMA) preferred. 2+ years experience as medical assistant required. Medical terminology knowledge.
