Position Overview

As a Hospice Aide, you will play a vital role in providing compassionate care to patients in their homes. Your primary responsibility will be to assist the primary care nurse in delivering high-quality patient care.

Key Responsibilities
  • Deliver patient care in alignment with the care plan established by the Hospice Care Registered Nurse and other interdisciplinary team members; execute job functions in a cost-effective manner consistent with the organization’s mission and vision; apply effective time management skills in organizing daily activities and fulfilling job responsibilities.
  • Assist with the emptying and documentation of patients' drainage collection devices; support patients with daily living activities such as bathing, grooming, oral hygiene, and skin care, while also providing perineal care, foot and heel care, and mobility assistance; participate in fall prevention measures, including necessary documentation.
  • Recognize and promptly communicate changes in patient conditions, particularly concerning pain and symptoms; report alterations in patient needs or conditions to the Hospice registered nurse or Team Manager; maintain accurate documentation of care provided in accordance with established policies and regulations.
  • Maintain a precise and up-to-date record of patients' Activities of Daily Living (ADL) and care provided, ensuring compliance with physician's orders and nursing plans; complete records according to agency guidelines, ensuring documentation is accurate and legible.
  • Perform essential light housekeeping tasks to support patient health; ensure cleanliness in patient areas by managing waste and maintaining a tidy environment; assist with linen management and organization.
  • Conduct daily supply checks; assist with inventory management; maintain temperature and refrigerator logs; provide care and assistance to patients during emergencies.
  • Alert nursing staff and team leaders immediately in case of emergencies; communicate effectively with patients, families, caregivers, and interdisciplinary team members.
  • Provide clear and timely verbal and written information regarding patient conditions and family concerns; demonstrate efficient use of supplies and equipment.
  • Consistently meet productivity standards set by the organization; participate in on-call rotations as necessary; attend scheduled in-service meetings and complete required annual training hours.

Qualifications
  • High School Diploma or equivalent is preferred.
  • Certification from a New York State Department of Health approved home health aide training program is required, along with a current and valid driver's license.
  • A minimum of one (1) year of experience in hospice, home care, or nursing home settings is preferred.
