You will be responsible for:
- Emptying and documenting patient drainage collection devices.
- Assisting patients with daily living activities, including bathing, grooming, oral hygiene, and skin care.
- Facilitating mobility through turning, positioning, and range of motion exercises.
- Maintaining a safe environment by participating in fall prevention rounds and documenting as required.
It is crucial to:
- Identify and report any changes in patient condition, focusing on pain and symptoms.
- Document care provided in accordance with established policies and regulations.
- Maintain accurate records of patient activities and care in line with physician orders.
Your responsibilities will also include:
- Performing light housekeeping tasks essential for patient health, ensuring cleanliness in patient areas.
- Managing supplies by checking inventory daily, assisting with orders, and maintaining organized storage.
In case of emergencies, you will:
- Alert the nurse and team leaders immediately.
- Communicate effectively with patients, families, and team members.
To ensure continuous improvement, you will:
- Meet productivity standards established by the agency.
- Participate in on-call rotations and attend mandatory in-service meetings.
To be considered for this position, you should have:
- A High School Diploma or equivalent.
- Certification from a New York State Department of Health approved home health aide training program.
- A valid driver's license.
- Preferred experience of at least one year in hospice, homecare, or nursing home settings.
