Patient Care Assistant

As a vital member of our healthcare team, you will be responsible for providing compassionate support to patients under the guidance of a primary care nurse. Your role is essential in ensuring the comfort and well-being of those in our care.
Key Responsibilities
- Deliver patient care in alignment with the care plan established by the Hospice Care Registered Nurse and the interdisciplinary team.
- Utilize effective time management skills to organize daily activities and responsibilities efficiently.
- Assist with the emptying and documentation of drainage collection devices, as well as support patients with daily living activities such as bathing, grooming, and oral hygiene, with a focus on skin care and mobility assistance.
- Monitor and report any changes in patient conditions, particularly regarding pain and symptoms, to the Hospice registered nurse or Team Manager, ensuring accurate documentation of care provided.
- Maintain meticulous records of patient Activities of Daily Living (ADL) and care, ensuring compliance with physician orders and agency guidelines.
- Perform light housekeeping duties essential to patient health, including maintaining cleanliness in patient areas and managing linen supplies.
- Conduct daily supply checks, assist with inventory management, and ensure the proper handling of medical waste.
- In emergencies, promptly alert nursing staff and communicate effectively with patients, families, and team members.
- Provide clear and timely information regarding patient conditions and family concerns, demonstrating efficient use of resources.
- Meet productivity standards set by the agency and participate in ongoing training and in-service meetings.
Qualifications
- High School Diploma or equivalent is preferred.
- Certification from a New York State Department of Health approved home health aide training program is required.
- A valid driver's license is necessary.
- At least one year of experience in hospice, home care, or nursing home settings is preferred.
Salary Information
The salary range for this position is $25,940-$37,660/year. Factors influencing salary may include location, specialty, service line, years of experience, education, and internal equity.
