Key Responsibilities
- Assist in delivering patient care according to the individualized care plan established by the Hospice Care Registered Nurse and the interdisciplinary team; maintain a focus on efficiency and alignment with the agency's mission and vision.
- Support patients with daily living activities including bathing, grooming, and personal hygiene, while ensuring comfort and safety; assist with mobility and physical exercises as directed.
- Monitor and report any changes in patient condition, particularly regarding pain and symptoms, to the Hospice registered nurse or Team Manager; document care accurately following established policies and regulations.
- Maintain thorough and up-to-date records of patient care and activities, ensuring compliance with physician's orders and agency guidelines.
- Perform essential light housekeeping tasks to promote a clean and safe environment for patients; manage linens and supplies effectively.
- Check and manage medical supplies daily, assisting with inventory and ensuring readiness for patient care needs.
- Respond promptly to emergencies, effectively communicating with the healthcare team and family members.
- Provide clear and timely updates regarding patient status and family concerns, ensuring efficient use of resources.
- Meet productivity standards set by the agency and participate in required training and meetings.
- Carry out additional duties as assigned, recognizing that all responsibilities are essential functions of the role.
- High School Diploma or equivalent preferred.
- Certification from an approved home health aide training program is required, along with a valid driver's license.
- Preferred experience of at least one year in hospice, home care, or nursing home settings.
The salary range and/or hourly rate provided is a preliminary estimate of potential compensation for this position and may be adjusted based on various factors such as experience, education, and internal equity.
