About the Role

We are seeking a skilled and compassionate Hospice RN to join our team at Enhabit Home Health & Hospice. As a Hospice RN, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Coordinate patient care and ensure continuous assessment of each patient's and family's needs.
  • Implement the interdisciplinary care plan and perform services in accordance with the physician's orders and the established plan of care (POC).
  • Ensure delivery of care is compliant with federal and state regulations.
Requirements
  • Must be a graduate of an approved school of nursing education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed registered nurse (RN).
  • Must have basic demonstrated technology skills, including operation of a mobile device.
Preferred Qualifications
  • Medicare home health or hospice experience is preferred.
Additional Requirements
  • Must possess a valid state driver license.
  • Must maintain automobile liability insurance as required by law.
  • Must maintain dependable transportation in good working condition.
  • Must be able to safely drive an automobile in all types of weather conditions.
  • Must possess CPR certification for the healthcare provider.
