As a Care Manager, you will be a vital member of our team, working closely with our Director of Value Based Care and other healthcare professionals to ensure that our patients receive the highest quality care.
Key Responsibilities:- Triaging and Coordination: Triaging, coordination, documentation, communication, and tracking of patient calls, cases, and records for a panel of medium acuity Traditional Medicare and Medicare Advantage Plan patients in the Chronic Care Management (CCM) Program.
- Patient Care Planning: Ensuring that patients with chronic conditions receive CCM services in an appropriate and timely manner.
- Comprehensive Care Plans: Establish and implement initial Care Plans, and review and revise Comprehensive Care Plans as needed.
- Quality Measures: Coordinate efforts with the Quality Department to ensure that each patient is accurately assessed, and that Annual Wellness Visits are completed and quality measures are met.
- Patient Engagement: Accurately update patient's problems list, diagnoses, health conditions, medications list, and Care Team, and bring additional medical concerns to the primary care provider as needed.
- Collaboration and Communication: Work closely with the Director of Value Based Care, the Lead RN Care Manager, and other CCM team members, providers, and Quality Improvement Manager to ensure that the care patients receive is efficient, thorough, timely, and properly documented.
- Education and Experience: Active Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) license. A minimum of 2 years of related care management, medical clinical experience, and/or training required.
- Skills and Abilities: Experience in social and care management services and patient engagement is required. Strong computer skills and proficiency in internet software and knowledge of Microsoft Office products is required.
- General Knowledge: General knowledge of primary care practice, clinics, knowledge of legal and ethical standards for the delivery of primary care, and medical terminology.
- Chronic Health Conditions: Well-versed in knowledge of chronic health conditions, acute care, behavioral health, and substance abuse problems.
- Communication Skills: Excellent verbal, phone, and written communication skills.
- Interpersonal Skills: Skilled in interacting with patients, staff, and external customers in a professional manner.
