Job Summary:

Wellbe Senior Medical is seeking a highly skilled Engagement Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to our geriatric patients in their homes. Job Description:

Key Responsibilities:

  • Provide comprehensive geriatric care to patients in their homes, including initial assessments, ongoing care, and coordination with other healthcare providers.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other members of the care team to ensure seamless transitions of care and optimal patient outcomes.
  • Conduct home visits to assess patient needs and provide education on disease management, medication adherence, and other health-related topics.
  • Order and interpret diagnostic tests, prescribe medications, and manage patient care in accordance with state and federal regulations.
  • Document patient interactions, assessments, and care plans in a timely and accurate manner.
  • Participate in regular team meetings to discuss patient progress, share best practices, and address any challenges or concerns.

Requirements:

  • Master's degree in Nurse Practitioner studies from an accredited program.
  • Active, unencumbered RN and NP licenses in the posted state.
  • Current board certification as an NP.
  • BLS certification and an active NPI number.
  • Active, unencumbered DEA licensure or eligibility to apply upon hire.
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics is strongly preferred.

Preferred Skills and Abilities:

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Experience with electronic health records and other healthcare software.
  • Ability to lift up to 30 pounds and move patients as needed.

Work Conditions:

  • Ability to work in a fast-paced, dynamic environment.
  • Ability to travel up to 100% locally to deliver patient care.
  • Ability to work in a variety of residential settings, including patients' homes.

Supervisory Responsibility:

This position will have no supervisory responsibility.

Travel Requirements:

Travel may be required up to 100% locally to deliver patient care.

Work Conditions:

Ability to lift up to 30 pounds. Moving, lifting, or transferring of patients may involve lifting of up to 50 pounds as well as assisting with weights of more than 100 pounds. Ability to stand for extended periods. Ability to drive to patient locations (e.g., home, hospital, SNF, etc.). Fine motor skills/Visual acuity.
