As a Registered Nurse Case Manager, your responsibilities will include:

  • Advocating for patients while collaborating with interdisciplinary healthcare teams, facilities, and families to ensure optimal care experiences from admission through discharge and beyond.
  • Connecting patients with resources to support their health and well-being.
  • Utilizing clinical expertise and independent judgment in assessing and reassessing patient needs, facilitating care coordination, managing utilization, and advocating for patients.
  • Ensuring compliance with medical appropriateness criteria for care status and levels.

Schedule: Days; 3-4 days per week; one winter holiday and one summer holiday required annually.

Qualifications:

To be considered for this role, candidates must possess:

  • A Nursing Diploma or Associate's Degree in Nursing; a BSN is preferred.
  • An active RN License in the applicable state of employment.
  • A minimum of 1 year of nursing or case management experience.
  • Preferred experience with Electronic Medical Records (EMR).
  • A solid understanding of regulatory requirements and accreditation standards is strongly preferred.
  • BLS certification from the American Heart Association is required.

Physical Requirements: Medium Work - Ability to exert up to 50 lbs of force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly.
