**About the Role**

Embrave is seeking a highly skilled and experienced Case Manager to join our team. As a Case Manager, you will be responsible for supervising adult felony offenders and developing and implementing case plans to support their rehabilitation and reintegration into society.

Key Responsibilities:

  • Supervise adult felony offenders and provide guidance and support to help them achieve their rehabilitation goals.
  • Conduct risk assessment interviews to identify potential risks and develop strategies to mitigate them.
  • Develop and implement case plans that are tailored to the individual needs of each client.
  • Coordinate and facilitate IRT client groups and provide counseling and support as needed.
  • Communicate effectively with courts, attorneys, probation officers, and parole officers to ensure that clients are meeting their court-ordered conditions.
  • Complete intake paperwork and verify employment for new clients.
  • Maintain accurate and up-to-date records of client progress and activities.
  • Make referrals to external agencies and services as needed to support client rehabilitation.
  • Prepare reports and provide updates to stakeholders on client progress.

Requirements:

  • Bachelor's degree in behavioral science or a related field.
  • Successful work experience in case management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Strong analytical and problem-solving skills.
