Key Responsibilities:

  • Utilize a variety of treatment approaches to work with clients and develop effective treatment plans.
  • Provide case management services, including assessment, planning, and coordination of care.
  • Collaborate with internal and external stakeholders to ensure seamless delivery of services.
  • Develop and maintain relationships with community health and mental health resources to ensure access to necessary services.
  • Conduct assessments and develop treatment plans that address the unique needs of each client.
  • Provide individual, group, and family therapy sessions as needed.
  • Document client progress and maintain accurate records.

Requirements:

  • Bachelor's degree in a human services field or related field.
  • Minimum of two years of experience in a human services field, preferably in case management or a related field.
  • Knowledge of mental illness, substance abuse, and psychopathology, as well as associated treatment modalities.
  • Working knowledge of Colorado Revised Statutes 27-10 (Mental Health Code) and HIPAA.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills.

Working Conditions:

The Case Manager will work in a clinical treatment team setting and will be required to travel between Mind Springs Health sites. The employee must be able to lift and/or move up to 25 pounds and must be able to sit for longer periods of time at a computer. The noise level in the work environment is usually moderate.
