PrairieCare is seeking a highly skilled Patient Intake Coordinator to join our Assessment and Intake Department. This role is responsible for handling external and internal requests for information and services for all levels of care at PrairieCare.
Key Responsibilities:- Complete Mental Health Screens over the phone with patients seeking services and schedule Admission Diagnostic Assessments and intake appointments for both program and outpatient clinic services as appropriate.
- Triage crisis phone calls, provide county crisis resources, and complete safety planning.
- Coordinate internal and external direct referrals to our services.
- Bachelor's degree in Social Work, Sociology, Psychology, or a closely related human services field.
- Two years of supervised experience in behavioral healthcare, meeting the criteria for a mental health practitioner or a license as a Licensed Social Worker (LSW) preferred.
- Experience in triage of psychiatric emergencies is preferred.
