About the Role:

We are seeking a dedicated and experienced LPN to provide skilled nursing care to patients in their homes. As an LPN, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing direct patient care. You will work collaboratively with our interdisciplinary team to ensure that patients receive comprehensive and coordinated care.

Responsibilities:
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Provide direct patient care, including administering medications, wound care, and other treatments as ordered by physicians.
  • Collaborate with the interdisciplinary team to ensure that patients receive comprehensive and coordinated care.
  • Communicate effectively with patients, families, and healthcare team members to ensure that patients' needs are met.
  • Document patient care and progress in a timely and accurate manner.
Requirements:
  • Current license as a Licensed Practical Nurse (LPN) in the State of MN.
  • 2 years' experience as an LPN.
  • Able to travel within geographic area serviced by the office.
  • CPR Certification.
