Overview

As an integral member of the PACE Interdisciplinary Team, the Enrollment Registered Nurse plays a crucial role in the assessment and enrollment/intake process of the PACE program. This position collaborates with the care team to ensure that participants meet the necessary qualifications and that the program's census remains within the required limits. Both new graduates and seasoned nurses are encouraged to consider this opportunity. The work schedule is from 8 AM to 4 PM, Monday through Friday.

Key Responsibilities

  • Conduct initial consultations to outline the PACE model to potential participants and their caregivers.
  • Verify that potential participants fulfill eligibility criteria in line with PACE enrollment regulations.
  • Utilize a strengths-based approach to assess the immediate needs and health status of potential participants.
  • Maintain communication with the care team regarding the status of prospective participants.
  • Balance and prioritize the needs of prospective participants and caregivers with the concerns of the care team.
  • Engage in weekly meetings to offer clinical insights, guidance, and support to the care team.
  • Collect and preserve accurate data on new referrals using CRM, EMR software, and MassHealth Virtual Gateway.
  • Conduct home visits to prospective participants to complete and submit the state-required Minimum Data Set-Home Care (MDS-HC).
  • Perform comprehensive needs and medical evaluations, including assessments of current functional status, caregiver involvement, existing services, and home safety.
  • Present and discuss initial needs assessment findings with the PACE Interdisciplinary Team.
  • Identify the necessity for and facilitate additional assessments for prospective participants.
  • Collaborate with the Interdisciplinary Team to evaluate the capability of prospective participants to safely remain in the community with PACE services.
  • Communicate enrollment status and updates to prospective participants and other involved parties.

Qualifications:

  • Possession of a current RN license in Massachusetts, or the ability to obtain one prior to the start date.
  • Current CPR Certification.
  • Frequent local travel with access to a personal vehicle.
  • Covid Vaccination and fit test are mandatory.
