Position Overview

As an integral member of the PACE Interdisciplinary Team, the Enrollment Nurse plays a crucial role in the assessment and intake process for the PACE program. This position collaborates with the care team to ensure that participants meet the necessary qualifications and that the program maintains its required census levels. Both new graduates and seasoned nurses are encouraged to consider this opportunity. The work schedule is from 8 AM to 4 PM, Monday through Friday.

This role requires in-person engagement and entails frequent local travel.

Key Responsibilities

  • Conduct initial consultations to explain the PACE model to potential participants and their caregivers.
  • Verify that potential participants fulfill eligibility criteria in accordance with PACE enrollment regulations.
  • Utilize a strengths-based approach to assess the current needs and health status of prospective participants.
  • Maintain communication with the care team regarding the status of potential participants.
  • Balance and prioritize the needs of prospective participants and caregivers with the concerns of the care team.
  • Engage in weekly meetings to provide clinical insights and support to the care team.
  • Collect and maintain accurate data on new referrals using CRM and EMR software.
  • Visit potential participants in their residences to complete and submit the state-required Minimum Data Set-Home Care (MDS-HC).
  • Perform comprehensive needs and medical assessments, evaluating functional status, caregiver status, current services, and home safety.
  • Present and discuss initial needs assessment findings with the PACE Interdisciplinary Team.
  • Identify the need for and facilitate additional assessments for prospective participants.
  • Collaborate with the Interdisciplinary Team to evaluate the ability of prospective participants to safely remain in the community with PACE services.
  • Communicate enrollment status and progress to prospective participants and other involved parties.

Qualifications:

  • Active RN license in Massachusetts or the ability to obtain one prior to starting.
  • Current CPR Certification.
  • Willingness to travel locally with access to a personal vehicle.
  • Covid Vaccination and fit test are mandatory.
