About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Case Manager. As a key member of our care team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Perform patient assessments and develop comprehensive care plans
  • Collaborate with the care team to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Document patient progress and outcomes
  • Participate in on-call rotations and provide after-hours support as needed
Requirements
  • One year of nursing experience
  • Current RN license, specific to the state(s) you are assigned to work
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
