Hospice RN Case Manager

At Interim HealthCare Staffing, we are seeking a skilled and compassionate Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in need.
Job Summary:The Hospice RN Case Manager will be responsible for maintaining a specified case management load, completing initial and ongoing assessments of patients' physical and psychosocial status, and developing and implementing individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and other healthcare providers.
Key Responsibilities:- Maintain a specified case management load and complete initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement individualized plans of care in collaboration with the IDG and other healthcare providers.
- Coordinate care among IDG members and other healthcare providers to ensure the continuous assessment of patients' needs and the implementation of the IDG plan of care.
- Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
- Direct and supervise the services provided by and LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
- Facilitate the consideration of another level of care when indicated by patients' or families' status or needs.
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina with an active and good standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
- If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
